Following is the Application format for reporting an Admin, if your application is not in this format or not answering all of these points then your application is rejected:

Name of the Admin you report:
Date of the Admin breaking a rule:
What does the Admin do (which is against the rules):
Which power does the Admin abuse or misuse:
Does the Admin help on you reports:
Proof of the person breaking a rule:
Screens of Admin breaking the rules :
